Chronic disease coordination services
Positions were funded between 2005 and 2008 to support the implementation of The Queensland Strategy for Chronic Disease 2005–2015.
Queensland Health chronic disease coordinators
Three chronic disease coordinator positions were created to assist and support:

Queensland Health Service Districts to develop, implement, maintain, monitor and evaluate Queensland Strategy for Chronic Disease 2005–2015 initiatives and programs

development of best practice models of service delivery and the use of evidence-based approaches to chronic disease prevention, early detection and management
-
facilitation of information sharing strategies
-
promotion of innovative initiatives in relation to chronic disease prevention, detection and management.
Non-government organisation chronic disease coordinators
Seven chronic disease coordinator positions were funded to support and build capability in chronic disease prevention, detection and management in:
-
culturally and linguistically diverse communities across Queensland through the Ethnic Communities Council of Queensland
-
Aboriginal and Torres Strait Islander communities through the Queensland Aboriginal and Island Health Council
-
general practice through General Practice Queensland
-
primary care agencies through Combined Health Agencies Group (CHAG).
