Alcohol and drug - funded positions

Alcohol and drug services

Positions were funded between 2005 and 2008 to support the implementation of the Queensland Strategy for Chronic Disease 2005–2015.

Alcohol and drug prevention workers

Alcohol and drug prevention workers aim to keep communities healthy by providing a range of primary prevention services focused on alcohol, tobacco and other drugs, and by promoting social and emotional health and wellbeing.

Alcohol and drug clinical workers

Alcohol and drug clinical workers deliver alcohol and drug treatment interventions to individuals and groups. Interventions are evidence based and in accordance with harm minimisation policy and practice, state and national health priorities, and local community priorities.

Nutrition and physical activity - funded positions

Nutrition and physical activity services

Positions were funded between 2005 and 2008 to support the implementation of the Queensland Strategy for Chronic Disease 2005–2015.

Nutrition and physical activity

Healthy Lifestyle Coordinators train and support facilitators to deliver group-based healthy lifestyle programs with a focus on healthy eating, physical activity, stress management and long-term weight management. Key lifestyle programs supported include:

Physical Activity Project Officers support physical activity promotion in mainstream and Indigenous specific programs.

Tertiary prevention and management - funded positions

Tertiary prevention and management services (including frequent and avoidable admission services)

Positions were funded between 2005 and 2008 to support the implementation of the Queensland Strategy for Chronic Disease 2005–2015.

Community Hospital Interface Program

The Community Hospital Interface Program (CHIP) teams focus on enhancing the transition between hospital and the community through coordination and care planning.

They work with clients on discharge from hospital to ensure continuity of care. Activities include early identification and intervention for at-risk clients who present or are admitted to the acute hospital setting, and in-home care for people when they are discharged from hospital.

Cardiac and pulmonary rehabilitation

Cardiac and pulmonary rehabilitation workers are members of multidisciplinary teams with outreach capacity. They provide evidence-based management programs and support for people with heart and respiratory disease and their carers.
