Service providers

The Queensland Strategy for Chronic Disease 2005–2015 recognises that in order to adequately address the burden of chronic disease there needs to be:

• an increase in health workforce capacity and infrastructure
• continued activity in the health workforce to ensure the recruitment, development and retention of an appropriately skilled workforce
• more flexible and innovative service delivery models
• health workforce realignment to meet changing patterns of service delivery and to respond to community expectations.

To support workforce capacity and realignment of service delivery, the strategy funded

three place-based initiatives

• Innisfail Healthier Great Greenway (PDF 30 kB)
• Logan-Beaudesert Health Coalition
• North Lakes and surrounds (PDF 39 kB)

additional positions or enhancements in the following program areas

• alcohol and drug services 
• nutrition and physical activity services 
• tertiary prevention and management services (including frequent and avoidable admission services 
• Community Hospital Interface Program
• cardiac and pulmonary rehabilitation
• chronic disease coordination
• telephone-based counselling services.
