In parallel with the development of the Queensland Strategy for Chronic Disease 2005-2015 is the development of three place-based initiatives in North Lakes and surrounds, Logan-Beaudesert, and Innisfail.
These initiatives focus on producing integrated service delivery models at the local level across the continuum of care.
The goal of the place-based initiatives is to develop new ways of working that engage and bring together a range of public and private providers to address the health needs of the local populations.
