Strengthening the system feature image of smiling womanPeople living with chronic disease are likely to meet a range of health service providers in a variety of settings across the health care system.

For example, a person with type 2 diabetes mellitus, in consultation with their general practitioner, may use community-based or outpatient services including an optometrist, diabetes educator, dietician, pharmacist and podiatrist. This person may also see specialists and require time in hospital if necessary. They may need pathology and diagnostic services, and they may have responded to public health or consumer health messages. People with a chronic disease often contact consumer support services seeking information on how to actively manage and participate in their health care.

Therefore, the health system as a whole needs to be able to respond to people experiencing or at risk of chronic conditions. The  Queensland Strategy for Chronic Disease 2005–2015 emphasises that collaboration is required at state, national and local levels to provide:
