How to search for Consent Forms & Patient Information

1. Click on the 'For Clinicians' button on the Informed Consent home page
2. Select the relevant sub-specialty
3. Search the list and click on the relevant link to download the pdf
4. Select 'File', 'Save As' to save a copy to your PC
5. If you don't know which sub-specialty to select, enter the name of the form into 'Search this site'
