Health
Outcomes Plan
Cardiovascular disease is specified as one of the National Health Priority
Areas. As a result the Health Outcomes Unit within Queensland Health has developed
the Health Outcomes Plan for Cardiovascular Health: Coronary Heart Disease.
The purpose of the Health Outcomes Plan for Cardiovascular Health: Coronary
Heart Disease is to identify for Queensland the outcomes which represent maximum
health gain and to identify and recommend evidence based strategies to achieve
these outcomes. This Health Outcomes Plan is based on the National Health
Outcomes Framework and provides an overview of the range of strategies which
empirical evidence and expert opinion suggest will result in improved cardiovascular
health for Queenslanders.
The plan identifies key action areas and strategies for the primary prevention, clinical management and secondary prevention/rehabilitation of Coronary Heart Disease, along with indicators of primary outcomes and risk factors. These strategies are proposed as State priorities for the prevention and management of Coronary Heart Disease.
