Interactive Free Text PDF HACC ONI (Ongoing Needs Identification Tool)Profiles.

The Interactive Free Text PDF ONI Profiles are an electronic option of the Ongoing Needs Identification (ONI) Tool.

The development of the Interactive Free Text ONI Profiles provides an alternative to the paper based ONI.

Please use only the files currently published on the HACC Website when conducting the ONI. Please do not use stored or saved older files and forms.

Improve legibility of ONI Profiles

Decrease time wastage of staff clarifying illegible referrals.

Improved timeliness of retrieval and updating of the ONI profiles.

Improve options for increased speed of input of data into service forms - especially when used over the phone.

Participate in electronic referral of the ONI commencement and sharing of ONI Profiles.

Information collected in the ONI is "CONFIDENTIAL". This information is not to be shared without consumer consent.

With consumer consent, information shared using this form can be via post, phone, face to face, fax or electronic methodology.

These forms can be downloaded, stored and used by any organisation, however;

Please note: Secure email location certificates (PKI) must be used when electronically sharing the ONI.

At this stage, assistance to enable secure email transfer capabilities for Queensland organisations to conduct electronic referral of the ONI will be confined to the E ONI Pilot sites (Gold Coast, Darling Downs and Southern Downs).


This pilot will be conducted from 1st February to the end of April in 2006.

The intention is that statewide rollout of secure email location (PKI) will be informed by the E ONI pilot and will commence April 2006

Computer requirements to use the Interactive Free Text PDF ONI Profiles

Minimum of 256 MB of memory on your computer

Ensure there is a copy of Acrobat Reader 7.0 ® installed on the computer.

Acrobat Reader 7.0 ® is available as a free download from www.adobe.com ( first link under 'Full Download') for Non-government agencies.

Government agencies (Queensland Health) need to contact their Help desk and log a request for the Adobe Reader program to be downloaded to their computer(s)
