Providing support and maintenance to those members of our communities who are frail aged or younger persons with a moderate to severe disability and their carers, is important in preventing inappropriate admission into residential care. Queensland Health has a key role in ensuring our consumers experience a health system which is transparent, consistent and responsive to their needs. Home Care Services will ensure consumers are better informed of their rights and responsibilities through collaboration and participation with consumers and their advocates in the planning, monitoring and evaluation of services.
To be eligible for Queensland Health Home Care Services the person must be an “older or frail aged person with moderate, severe or profound disabilities or younger person with moderate, severe or profound disabilities whose capacity for independent living is at risk of premature or inappropriate admission to long term residential care”. The person must have a current Aged or Disability Australian Pension Concession Card (PCC) issued by Centrelink or Department of Veterans Affairs.
Queensland Health Home Care Services are funded by the Home and Community Care (HACC) Program to provide a range of flexible, responsive and coordinated, basic support and maintenance services that assist eligible people to maintain independence in their home.
