What's new

The role of Home and Community Care (HACC) service providers is to assist clients and their carers function to the best of their ability at home and in the community by providing support services appropriate to the clients needs.

Assistance and support is targeted at all HACC eligible clients in need of service providers, regardless of their ethnic background or language spoken.

There are a wide range of services to support independent living in the community. The Commonwealth Respite and Carelink Centre has explained the different community care services (packages and programs) available.

Service Agreements – from 1 July 2010
Organisations that commence an agreement from 1 July 2010, to deliver services, will need to enter into a Small Grants Agreement or a Service Agreement. The Service Agreement Package can be viewed at Service Agreements – from 1 July 2010 - Community Services (Queensland Government).

  • Carers Queensland
    Provides carer support, and group/telephone counselling for carers. Offices in Brisbane, Gold Coast, Toowoomba, Dalby, Maroochydore, Hervey Bay, Rockhampton, Mackay, Townsville and Cairns.

  • HACC Service providers:  HACC and Community Health allied health contacts state wide are found in the Service Directories.
  • Commonwealth Carelink:  national database has information about allied health services, including day therapy centres). 
  • Multicultural Affairs Queensland (MAQ):  can refer clients with a migrant or refugee background to the appropriate professional physical and/or mental health or social support agencies that specialize in this area.
