Heart Failure Medication Problem Solving Guidelines

The Heart Failure Medication Titration Problem Solving Guidelines are endorsed by the Queensland Statewide Heart Failure Service Steering Committee and have been designed to complement the Statewide Medication Titration Plan.

Instructions on Use of Form

The Heart Failure Medication Titration Plan is designed to be generated by a Heart Failure Service (HFS).

Each HFS is able to insert HFS name, contact details and change filing instructions, then save it.  If you wish, you have the ability to fill individual patient forms electronically as well, and save and email it.  You are encouraged to file the original in the medical notes and send a copy to the GP, even if your site is managing the titration.  This gives the GP the problem-solving guidelines and the variable diuretic plan.

NB. The form is only endorsed for titration by GPs, Heart Failure Nurse Practitioners or Heart Failure Nurse Practitioner candidates.  Autonomous CNC titrating medications is beyond scope of practice and requires Medical Officer sign off for any changes to medication.
