End-Stage Heart Failure Protocol and Guidelines

The Prince Charles and Royal Brisbane and Women's Hospital; and associated community services in Brisbane, have developed a management protocol and guidelines to support the needs of patients in End-Stage Heart Failure. The development of these tools was supported by the Australian Commonwealth's Department of Health and Aging, Local Palliative Care Grant, (2008-2009).
