Queensland Heart Failure Services

Since 2005, Queensland Health has made a commitment to establishing multidisciplinary teams of nurses and allied health to help patients with heart failure better manage this long term condition. These teams are supported by medical staff.

Heart Failure Service Ingredients

Queensland Heart Failure Services have specialist heart failure nurses and some also have dedicated pharmacists, physiotherapists, exercise physiologists, occupational therapists, dietitians, social workers and psychologists. Services offered usually include some or all of the following:

  1. Follow-up: home visits, telephone, clinics;
  2. Medical follow-up;
  3. Patient and carer education;
  4. Self-management strategies;
  5. Weight monitoring;
  6. Dietary advice;
  7. Exercise;
  8. Medication review; and
  9. Social and psychological support.
