Student diary information

Feedback form for schools
After the vaccination clinic at your school, you'll be given a feedback form to provide any comments. Your feedback helps Queensland Health to improve future school based vaccination programs and address any issues encountered during the program.
The School Based Vaccination Program Resource Kit for Vaccine Service Providers provides guidelines, checklists, materials for distribution and suggestions on clinic layout. It should form the basis for development of vaccine service provider policies and procedures.
