Please note: A Hospital Autopsy Information Sheet for Staff is available on QHEPS. To locate:

  1. Open QHEPS via the Internet Explorer icon on your desktop
  2. From the Menu select the Clinical TAB and then select QHPSS listed under the CLINICAL SUPPORT heading
  3. Select Autopsy Informed Consent listed under the General Information Heading
  4. Select Hospital Autopsy Information Sheet for Staff
