Patient Consent Forms

The following procedure specific consent forms have been made available for patients, medical practitioners and other health care providers to access.

These consent forms have been grouped according to the type of operation to be undertaken. Just click on the category on the left which applies to your operation, and you will be taken to a listing of matching consent forms.

The Information sheets - "Be Informed - 12 Questions to Ask About Your Surgery", "Anaesthetic - Patient Information", and a general consent form are also available in eight languages other than English, including Arabic, Bosnian, Chinese, Croatian, Farsi, Serbian, Spanish and Vietnamese.
