STATEWIDE PRESCRIBER PROCEDURE MANUAL - Refer to the General Information and updated Continence Aids Sections in the MASS Statewide Prescriber Procedures Manual for information on the new subsidy levels and specific eligibility criteria.
STANDING OFFER ARRANGEMENT - MASS implemented a Standing Offer Arrangement (SOA) for continence aids on 1 February 2007. The Approved Continence Aids List (updated September 2009) and Continence Aids Supplier List provide information for prescribers on the SOA products.
FORMS - Prescribers use the MASS 50 application form for initial and two-yearly clinical assessment applications and MASS 51 Clinical Justification form (if needed). MASS clients use the MASS 55 re-application form. If you hand write your forms you may prefer these versions - MASS 50, MASS 51 or MASS 55.
