FORMS & INFORMATION - Use the MASS 20 application form for daily living aid applications, or the MASS 20 DLA short form (refer to the user guide). The new MASS 20 URGENT form will need to be completed if you are requesting urgent processing justification. Use the new Hoist/Sling Compatibility Checklist if different brands are requested.
