MASS - Mobility Aids

Updated MASS Mobility Aids Procedures and Forms come into effect 1 May 2008 - the equipment update provides a summary of these changes. Refer to the General Information and Mobility Aids Sections in the MASS Statewide Prescriber Procedures Manual for specific eligibility criteria.

MASS implemented a new Standing Offer Arrangement (SOA) for wheelchairs and wheeled walking aids on 1 June 2006. The SOA Approved Mobility Aids List, Mobility Supplier List, and Mobility SOA Information spreadsheet provide information for prescribers on the new SOA products.

Complete the updated MASS 20 for mobility aid applications, or use the updated MASS20MWC or MASS20WWA short forms referring to the user guide to clarify when these may be used. The new MASS 20 URGENT form will need to be completed if you are requesting urgent processing justification. The MASS 85 Permanent Loan Aid Information Sheet has been developed for applicants who use plaqued items of equipment.
