NEW 2009 MOBILITY STANDING OFFER ARRANGEMENT (SOA) COMMENCES 1 JULY 2009 FOR WHEELCHAIRS AND WHEELED WALKING AIDS (updated 12 October 2009) - The 2009 SOA Approved Mobility Aids List, Mobility Supplier List, product SOA spreadsheet or product table for information on the new SOA products and suppliers.
FORMS & INFORMATION - Complete the MASS 20 application form for mobility aid applications, or use the MASS20MWC or updated MASS20WWA short forms referring to the user guide to clarify when these may be used. The MASS 20 URGENT form is required if you are requesting urgent processing justification. The MASS 85 Permanent Loan Aid Information Sheet for applicants who use plaqued items of equipment.
