Universal Postnatal Contact Services
The Queensland Government has allocated funding to ensure that all women have access to follow-up contact from a health professional after the birth of a baby.
Building on work already being undertaken by maternity and child health services, the three aims of the initiative are to:
AIM 1
Develop a universal platform for pregnancy and parenting support, including early intervention, health promotion and education.
AIM 2
Promote continuity and integration of care between maternity services and community service providers.
AIM 3
Develop sustainable, collaborative maternal and child health community partnerships.
These aims will be achieved through:
- Integrating the Child Health Line with 13 HEALTH to provide 24-hour free and confidential telephone advice for new parents
- Universal and routine screening for key risk factors to identify families who may need additional support and/or referral to an appropriate service
- Developing service networks and partnerships with community based health service provider/s including GPs, non-government and community organisations to enhance care pathways
- Expanding home visiting and post-natal follow-up services to all women who give birth in public maternity facilities
- Providing a seamless transition of care between hospital and community settings, including through the establishment of Newborn and Family Drop-in Services
- All Queensland Health maternity facilities have received funding under the initiative through a service agreement with the Primary, Community & Extended Care Branch
