Patient harm or near misses are reported to the Patient Safety Centre, Centre for Healthcare Improvement.
The goal of clinical incident management is to prevent patient harm. It is an essential component of quality patient care and is achieved by having processes that:
A 'clinical incident' is any event of circumstance which has actually or could potentially lead to unintended and/or unncesessary mental or physical harm to a patient of a health service facility. Clinical Incidents include adverse events (harm caused) and near misses (no harm caused).
All clinical incidents should be reported promptly using the Queensland Health statewide clinical incident management information system - PRIME Clinical Incidents (CI). This applicaiton can be found on every Queensland Health desktop computer via the 'reporting concerns' icon.
The process for managing and reporting clinical incidents within Queensland Health is described in the Queensland Health Clinical Incident Management Implementation Standard.
