Application Protocol Requirements

What application forms do I need to submit?

All applications must submit the NEAF (National Ethics Application Form). This can be found at http://www.ethicsform.org/au/ 


Princess Alexandra Hospital Site Specific Requirement
There are specific requirements of the PAH Human Research Ethics Committee and the PAH Commercial Business Unit (coming soon).
 
The following forms must be completed and returned with your application:

o         Where research projects place demand on the resources of various PAH Departments such as the Departments of Radiology or Pathology, beyond that which is required for the routine clinical management of patients, the extra requirements must be approved by the Director of the relevant Department(s) and a Resource Form must be completed, signed and submitted to the Ethics Secretariat.

o         All approvals for Pathology services should be signed by Dr Kee Cheung (on behalf of Dr Michael Wiley, the Director of Queensland Health Pathology Services). Phone 3240 2398.

o         All trials involving drugs must be notified to the Director of Pharmacy to ensure that there is a central record of drugs being used in research. Agreement should be reached with regard to storage and handling of items involved. The signature of the Director of Pharmacy will be required on the Resource Form
