Human Error And Patient Safety (HEAPS)

What is HEAPS training?

 

Human Error And Patient Safety (HEAPS) training was originally developed by the ErroMed Group to introduce health care workers and managers to human error as a contributing factor to patient harm in the workplace. The training course aims to promote learning from human error as a valuable strategy to reduce preventable patient harm and assist in creating a more reliable and safe system of health care. The fundamental principle of HEAPS is to change traditional attitudes that health care workers have around ‘blame and train’ to a more constructive and proactive approach of identifying and learning from human error as a tool to improve patient safety.

 

HEAPS training assists staff at all levels to:

 

• Understand error chains and analyse systems of work rather than just blame the individual
• Realise the critical importance of communication and teamwork to patient safety
• Acknowledge that human error is inevitable and even the best staff can be involved in some of the worst outcomes
• Identify the need to discuss adverse events openly and learn from them
• Begin to move away from the ‘blame culture’ and access the means to do this
