Population Health Branch
Population Health Branch is a statewide service within the Division of the Chief Health Officer which is located in the Health Services Directorate alongside the 20 health service districts.
Population Health Plan
The inaugural five year plan for Queensland Health’s population health function was launched in November 2007. The Queensland Health Population Health Plan 2007-2012 is a critical mechanism for achieving our aim to:

deliver the best population health services nationally
-
improve the health and wellbeing of the population
-
reduce the health status gap between the most and least advantaged in the community.
The Plan is a key strategic document for Queensland Health’s population health function. It articulates the directions for population health for the next five years and major bodies of work to progress. The directions outlined in the Queensland Health Population Health Plan 2007-2012 expand on the department’s vision for health system reform outlined in the Queensland Statewide Health Services Plan 2007-2102, which was released in February 2007.

Key Functions
Population health is distinguished from other roles of the health system by its focus on the health and well-being of populations, rather than individuals. The objectives of population health are:

protecting health
-
preventing disease, illness and injury
-
promoting health and well-being.
Population Health Branch is responsible for:
-
coordinating and providing leadership for statewide public health planning, strategy development, implementation, monitoring and evaluation

implementing, coordinating and supporting public health programs for priority health issues of statewide and local significance

undertaking health surveillance and disease control initiatives including responding to disease outbreaks
-
implementing or overseeing the implementation of public health legislation.
Population Health Branch achieves this by working collaboratively with and supporting a range of organisations including other health services; other government departments; local government; non-government agencies; research institutions and local communities.
