Each private health facility must submit details of any death which was not reasonably expected to be the outcome or incidences of serious harm which occurred to a patient in the facility.
Root Cause Analysis
A Root Cause Analysis, if required, is conducted to identify and improve the procedures or practices relating to the provision of the health service that contributed towards the occurrence of the incident, rather than on the conduct of individuals.

The collection of 6 monthly adverse outcome data from each licensed private health facility is intended to identify specific aspects of the health service provision that may require examination or closer monitoring. The adverse outcome data collection is not intended as a method of benchmarking the quality of health services from one facility to another. These reports must be given within 35 days after the end of each 6 monthly period.
