Obtaining services

To access services through QALS you must be registered with us. This is a simple process and normally completing a single form is all that is required. If you have been compensated or are seeking compensation for your loss then some additional paperwork may be required.

New amputees are referred on to QALS from an amputee clinic after assessment by a Multi-Disciplinary Team (MDT). This flowchart shows the normal process for new amputees.
