: Instructions for private hospitalsIf the PICQ2008TM software is to be installed at different private hospitals of the same group, each hospital (or an individual representing each hospital) must complete a PICQ2008TM application form.
STEP 1: Individuals from private hospitals complete the PICQ2008TM private hospital application form. The application form is necessary to:
Users from private facilities are required to read and agree to the terms and conditions of the PICQ2008TM Licensing Agreement. [
19kb] Therefore, when registering for this software you will be required to indicate agreement to the PICQ2008TM Licensing Agreement.
It is most important that users note that the following commitments are within the License Agreement:
As outlined in clause 2 of this agreement, the licence grants users who have registered for PICQ2008TM (via the PICQ webpage) the right to install and use PICQ2008TM software on their PC hard drive.
The licence restricts copying the PICQ2008TM Software. Please refer to clause 3 where it is outlined that each individual user is allowed to make one copy of the PICQ2008TM on their hard drive and one as a back up. PICQ2008TMmust not be saved on a network drive.
It is important that you use PICQ2008TM for its intended purpose. The licence has restrictions on the use of PICQ2008TM software. Restrictions are outlined in clause 4 and notes that users must not decompile, reverse engineer, disassemble, modify, adapt, translate, rent, lease, loan or create derivative works based on the PICQ2008TM software or part thereof. You also may not transmit PICQ2008TM software over a network or from one computer to another.
When using PICQ2008TM, it is important that users are aware of legislation referring to patient confidentiality and privacy especially in regards to the utilisation, sharing and transmission, of patient level data.
STEP 2: Once your PICQ2008TM application form has been submitted, the Statistical Standards Unit (SSU), Health Statistics Centre (HSC) will receive and process the application.
If the form is complete and the individual has acknowledged and agreed to the terms and conditions of the Licence Agreement, the SSU will email you the PICQ2008TM software and related procedures.
