: Instructions for Queensland Health public hospitals and Health Service DistrictsIf the PICQ2008TM Software is to be installed at different campuses of the same hospital, each individual campus (or an individual representing the campus) must complete a PICQ2008TM application form.
STEP 1: An individual representative from a Queensland Health public hospital or Health Service District (HSD) completes the PICQ2008TMapplication form. This form is required to:
The Health Statistics Centre (HSC), Centre for Healthcare Improvement (CHI), have accepted the licence conditions on behalf of public hospitals & Health Service Districts. Although the licence has been agreed to on your behalf, users should acknowledge the terms and conditions of the PICQ2008TM licensing agreement. [
19kb] Therefore, when registering your request for PICQ2008TM you will be asked to indicate your acknowledgement to the PICQ2008TM Licence Agreement.
It is most important that users note that the following commitments are within the License Agreement:
The licence grants users who have registered for PICQ2008TM (via the "PICQ" webpage) the ability to install and use PICQ2008TM software on your PC hard drive accessible by the registered user. PICQ2008TM must not be installed on a shared network drive.
The licence restricts copying the PICQ2008TM software. Please refer to clause 3 where it is outlined that each individual user is allowed to make one copy of the PICQ2008TM on their hard drive and one as a back up.
It is important that you use PICQ2008TM for its intended purpose. The licence has restrictions on the use of PICQ2008TM software. These restrictions are outlined in clause 4 and state that users must not decompile, reverse engineer, disassemble, modify, adapt, translate, rent, lease, loan or create derivative works based on the PICQ2008TM software or part thereof. You also may not transmit PICQ2008TM software over a network or from one computer to another.
When using PICQ2008TM, it is important that users are aware of legislation referring to patient confidentiality and privacy especially in regards to the utilisation, sharing and transmission of patient level data.
STEP 2: Once your PICQ2008TM application form has been submitted, the Statistical Standards Unit (SSU), Health Statistics Centre (HSC) will receive and process the application.
If the form is complete and the individual has acknowledged and agreed to the terms and conditions of the Licence Agreement, the SSU will email you the PICQ2008TM software and related procedures.
