| Make payment directly to QHPSS using any of the payment methods shown below, then take your receipted invoice to a Medicare office to claim your rebate. If these services were provided whilst you were in hospital, the balance may be claimable from your private health insurer. | Detach the 'Payment Advice' from your account and send the top portion of the invoice to Medicare with your completed Medicare form. Medicare will send you a cheque which you can then forward to QHPSS together with the Payment Advice, the bottom half of the Medicare statement of benefit and any additional amount due. |
