Accreditation certification

To apply for an accreditation certificate, the following steps should be followed:

Refer to the following information sheet for specific details relating to the area in which accreditation is requested.

Step 3

Submit the completed application form, with verification of qualifications and experience as requested by the accreditation criteria information sheet, to Radiation Health.  The prescribed fee must accompany the application.
