Guidelines for Referring Patients

These guidelines have been produced to provide service providers with a concise reference of the referral systems available and offer an efficient approach to access services appropriate to patients needs.

Generic Referral Form

A generic referral form has been developed in collaboration with various groups and aligned with the General Practice Advisory Council (GPAC) Continuity of Care Planning Framework. It is available for downloading into General Practitioner's (GPs) clinical software by accessing the Metro North Brisbane Medicare Local website.

Process
The use of the generic referral form is preferred to efficiently provide adequate referral content to allow categorisation, prioritisation and direction of patients to appropriate services.  In most cases, referrals should be faxed or posted to the relevant department with a copy given to the patient.

If a patient's condition requires an urgent referral to a particular specialty, the registrar or specialist in the relevant discipline should be contacted.

Emergent Referrals

Local patients who require a comprehensive rapid assessment as a result of acute or unstable conditions should be referred to the Department of Emergency Medicine, Ground Floor, Dr James Mayne Building.  For medical advice or advice on alternative means of management ring the Department of Emergency Medicine Physician on call (07) 3646 5900.

Process
A referral letter should accompany the patient or faxed to (07) 3636 8723. If it is necessary to advise the department of the patient's impending arrival, please contact triage on (07) 3646 7109.
