Step 1
|
Gain a referral from a medical officer such as a General Practitioner.
|
The referral should include:
- your child’s name and date of birth and residential address
- both parents full names, residential address, and all phone numbers, such as home, mobile and business numbers (if a parent’s name is different from the child’s name please include your full names)
- reason for referral and summary of child’s medical condition
- treatments and/or investigations previously carried out on your child
- summary of results as appropriate

Step 2

Post or fax your referral to:

Step 3
|
When the RCH Outpatients department receives your referral, it will be assessed and given an urgency ranking for an appointment.
|
|
|
Step 4
|
The RCH Outpatients Department will
- telephone you regarding the appointment
- Send you an appointment in the mail, or
- Address the referral concern in a letter to the doctor that referred you.
|
If you have not heard anything about your child’s appointment within three (3) weeks of sending a referral, contact the Outpatient Service on 07 3636 7818.
|
A full medical history will be taken at the appointment including immunization details. Please have this information at hand.

My child has been to Outpatients before. Do I need a new referral?
You need a referral if:
- your child has not been seen within the past 12 months by the same specialty area
- your child is being seen for a different specialty area (e.g. your child was seen by a respiratory specialist and now needs to be seen by a renal specialist).
You do not need a new referral if:
- your child has been seen within the past 12 months for the same specialty area.

How do I find out about my child's appointment?
The RCH Outpatients Department staff will either:
- telephone you regarding the appointment
- send you an appointment in the mail, or
- address the referral concern in a letter to the doctor who referred you.
