Referral forms and templates

Using standardised referral formats is a simple way to create and transfer referral information to the Royal Children's Hospital. It supports our GP network by ensuring relevant data is included to progress your referral in a timely way.

Importing the template into your practice software will allow you to populate the majority of the template automatically. Instructions are kindly provided by GPpartners.

Referrals can be sent electronically using either HealthLink or Medical Objects. This has the following benefits:

• timely receipt of your referral into a dedicated RCH referral centre
• acknowledgement of referral receipt from Queensland Health
• the ability for RCH to communicate electronically regarding any further information required.
