Getting assessed

If you think it’s time to look into aged care, you’ll need to have an assessment.

You need to be referred for an assessment by a social worker, or another one of our health workers. You can also do this through the Australian Government’s My Aged Care website.

If you're in hospital, we'll do this for you. If you're at home and need support, call us on 07 4043 0100 or the local Cooktown Medical Centre to refer you to a social worker.

Come and see us

We encourage prospective residents and their families to visit and look at our facilities. If you’re interested in visiting and meeting our team, call us on 07 4043 0106.
