Accessing Your Information

Under the Queensland Freedom of Information Act 1992 (FOI Act), patients have the ability to access information regarding their treatment at any of our facilities.

How to make an application

Your application shoud be in writing and forwarded to the FOI decision-maker at the facility to which your application refers. Contact details for the major facilities in the district are listed below:

What can you ask for?

You can ask for any documents held by Queensland Health. This includes any documents contained in your patient record, regardless of whether it is paper or electronic.

How much does it cost?

Access to documents containing information about your own personal affairs is free. An application fee of $36.50 is payable when you are seeking access to documents which do not concern your personal affairs, and a cheque / money order for this amount must accompany your application.

Where photocopies of non-personal affairs documents are requested there will be a charge of 20 cents per page for all A4 copies provided. Should you request documents in any other format, you will be charged the actual cost for production.

Proof of identify

A proof of identity will be required before you will be granted access to your personal information. Appropriate documents include:

  • Original birth certificate
  • Certified copy of an extract of your birth certificate
  • Current Australian passport
  • Current Queensland or interstate driver's licence or learners permit
