GP Integration

Rationale
General practice and the Hospital sector both play an important part in the provision of services to people with a chronic disease. General Practice is generally the initial point of contact for patients with a chronic disease. It plays a key role in prevention, diagnosis and management of chronic disease and utilises the expertise of the hospital sector services as part of the provision of quality care. Unfortunately General Practice and the Hospital sector have become over the years increasingly isolated from each other resulting in fragmentation of care.

Intent
To improve the management of chronic disease and its risk factors and increase consumer satisfaction with the health system by developing an integrated system of care delivery through the collaborative action of the public-private sector

Goal
To achieve integration this initiative will work at developing innovative models to enhance the continuum of care between Southside Health Service District (Logan Beaudesert Area) and GPs, focusing on strategic developments, communication systems and health service delivery.

Objectives
1. To facilitate the development of a positive culture between general practice and the public health sector
2. To improve transitions between the Public Health Sector and General Practice
3. To ensure Health care is provided in the most appropriate setting

The Program
The GP-Integration program works in partnership with GP’s, Networks of General Practice and Queensland Health (Southside HSD) to identify, prioritise and implement models of care in response to the Southside Health Services Plan. Programs will be responsive to community needs and may change over time.

Staffing

  • 1 FTE GP-Liaison Officer
  • 0.5FTE GP Consultant
  • Administrative support will be provided by the Administration Officer LBHC
