Release of Information

According to the GPAC Privacy Guidelines (which were developed in alignment with the Health Services Act 1991), "information being provided by a QH health professional to a GP does not require QH staff to view a completed patient consent form.  Where this information is being provided by a QH staff member other than a health professional, the GP is required to provide evidence of patient consent. The form of consent required in this situation is determined at a local level and will be communicated to the GP."

To release patient information for a patient being treated within the Southside Health Service District, patient consent must be received.  This may be verbal or written consent.
