Social Work

Reasons for referral to the Social Work service might include:

• Difficulties with family conflict
• Concerns and issues for families and carers
• No family / social support for patient
• Patient experiencing difficulties coping with illness / disability
• Grief issues - death / significant loss
• Assessment or arrangement of ongoing care, e.g. ACAS / interim care
• Allegations of elder abuse / neglect or family violence
• Assistance in guardianship administrator applications (when there are no alternate decision makers)
