As part of the Falls Management and Prevention Project developed by the Brisbane South Centre for Health Service Integration, a best practice referral checklist was developed for health professionals.
The referral checklist aims to assist health professionals to refer older people living in the community who have fallen to suitable health care providers from different disciplines and agencies, with a view to facilitating better management of people who have had falls.
The checklist is consistent with Queensland Health's Falls Prevention Best Practice Guidelines for Public Hospitals and State Government Residential Aged Care Facilities (2003).
