These materials are resources to help you to develop a plan of action to implement effective falls prevention strategies. These materials support the activities of Phase 2 of the Toolkit.

Decision making
Making decisions about what to do to prevent falls and promote healthy active ageing is not always easy. Decision making needs to be based on the benefits, capacity, potential risk, acceptability to the target group, costs and supporting evidence.

Key stakeholders are people who can give you information about what is already happening in the community, and who can be involved and support your work.

Roles and responsibilities
As there will usually be a wide range of stakeholders, it is useful to define the roles and responsibilities different key stakeholders will take in reducing falls and promoting healthy active ageing.

Good working relationships
A successful project/program is based on developing and maintaining good working relationships. This includes communicating well with others and problem solving.

Partnerships
Partnerships are defined and explored, with a focus on how partnerships contribute to falls prevention program and how to encourage long-term and productive partnerships.

Community planning meetings are used to engage stakeholders, and include introductory meetings, 'getting started' meetings, planning and progress meetings.

This section outlines what works to prevent falls from happening, according to scientific evidence.

This section discusses a community or population based approach to falls prevention, including addressing multiple risk factors, a life course approach and a healthy active ageing approach.

WHO Safe Communities
This section discusses the World Health Organisation's (WHO) Safe Communities model and how it can be applied to falls prevention and healthy active ageing.

Falls risk assessment
This section discusses how conducting an individual falls risk assessment and implementing appropriate interventions can assist in preventing or minimising falls.
