Clients accessing the wider community.
It is vital that clients have regular access to the wider community. Our Community Links Program assuits clients to the wider community. This can can be specific community access outings and leisure outings, usually associated with a discharge plan which also invloves ensuring clients are familiar with theri area of discharege, including familarising themselves with relevant services.
