Overview

The Townsville Community Care Unit (TCCU) is a 24 bed residential facility located at the Kirwan Health Campus. Accommodation is provided in single and two bedroom apartments. The unit functions within a recovery model, with a strong focus on return to community living and community inclusion.

Consumer programmes are individualised and include livings skills, recreational and leisure activities, medication education, budget management, and information about dual diagnosis and assistance with accessing education or training and employment. Programs at Townsville Community Care Unit (TCCU) are recovery focussed and clients and family or carers participate in programme planning and review of their achievements.

Consumers are encouraged to work with community based services and Non-Government Organisation supports. Families and carers are included where appropriate in discharge planning. Rehabilitation multidisciplinary team members work with consumers, carers, NGO's and community case managers to effect a successful transition to community living.

Referral Procedure

Referrals are accepted from across the Northern Area Clinical Network. Assessment criteria, referral packages, patient information brochures, referral pathways and all enquiries can be obtained by contacting the service.
