Extended Acute Care

Wound Management Service

The wound-management service offers patients the opportunity to receive short-term, post-acute wound care provided by the staff from Townsville Community Health Service after the patient has been discharged from the hospital. This service is available at the clinic located at Kirwan Health Campus, or as an outreach service where the nurse arranges home visits at no cost to you.

Domiciliary Home Service

Domiciliary home service offers patients the opportunity to receive short-term assistance with hygiene and medication prompts. This service arranges a nurse to visit the patient in his/her home (or discharge destination) and may assist with referral for ongoing services in the community.

Home Intravenous Therapy

Home intravenous therapy offers patients who meet medical and nursing criteria the opportunity to receive out of hospital intravenous therapy as an alternative to treatment in hospital. This service is available in the clinic room located at the Kirwan Health Campus or as an outreach service where the nurse arranges home visits at no cost to you.

EAC services work within boundaries which remain flexible and negotiable to meet the needs of the consumer for the best outcomes and include the following,

Discharge Coordinators:

Discharge planning services are facilitated by four discharge coordinators who have specific areas (medical, surgical, oncology, pre-admission clinics, emergency department, rehabilitation unit and mental health). All patients requiring community services are flagged with the discharge coordinators who will assess, organise and facilitate discharge-planning needs for the patients in consultation with allied health, nursing and medical staff. Community services will then be arranged according to the patient's identified needs and requirements.

A veteran liaison officer with similar responsibilities is also available. The VLO is not only a liaison contact for The Townsville Hospital but for the Townsville Health Service District.

Day of Discharge Lounge

This service caters for all clients awaiting discharge requirements to be finalised or picked up from the hospital by QAS, family or other community transport services. Clients are offered a comfortable non-clinical waiting area. Music, TV, beverage making facilities and light snacks are available.

Referral Procedure

Referrals are accepted from within the Townsville Health Service District and external providers. The majority of referrals are from within Queensland Health services, but it does include referrals from the Mater private hospitals and general practitioners for the out-of-hospital IV service and wound clinic. Written referrals are required and verbal negotiation can occur prior to referral being sent to ensure the patient meets the criteria for service being requested.

Referrals for Discharge Coordinators

Referrals are accepted by phone.

Research

Each clinical nurse has an annual research project linked to his/her clinical area of expertise.
