- Health protection, health promotion, illness prevention and e4arly detection at individual and population levels;
- assessment, treatment and referral at the first contact point in the health system
- community-based management for people with chronic and complex conditions
- community-based health maintenance support for people with a disability and frail older people living independently in their own environment
- community-capacity building
