Provide Evidence of Identification (EOI)

This guideline describes the Evidence of Identification (EOI) requirements for the Location and the Duly Authorised Officer (DAO).

Note: Details of business and individual EOI requirements are provided as part of the online application process.

Provide Evidence Of Identification (EOI)

For Site Certificates and Certificates to be issued, Evidence of Identification (EOI) must be provided for both the Location and the Practice or Pharmacy staff member appointed as the Duly Authorised Officer (DAO).

Note: Medicare Australia Site Certificates are being used rather than Individual Certificates. In law, however, Evidence of Identification must relate to the individual behind the entity.

Certified copies of EOI documentation must be forwarded to Medicare Australia.

Business HSE Documentation

The Practice or Pharmacy must furnish Medicare Australia with Evidence of Identification material that verifies the identity of the business entity. The minimum requirement is a single document that may be one of the following:

Individual (DAO) Documentation

The Practice or Pharmacy officer appointed as the DAO for the Location must also provide EOI documentation, with a minimum of 100 EOI points required.
