Resignation / Transfer of HCL Representative
This guideline should be followed
if a HCL Representative resigns or moves elsewhere in the organisation.
Resignation or transfer of HCL Representative
- If a HCL Representative leaves the
Practice or Pharmacy for which the HealthCare Site Certificates have been
issued, they must inform Medicare Australia of the changes and send a letter, (on
Practice / Pharmacy letterhead,) identifying the name of the new appointee.
- If the DAO resigns, a new Agreement must be submitted to Medicare Australia, the
current keys and certificates revoked and re-issued, and a new 100 point EOI
check for the new DAO submitted.
- If the HSE Rep resigns, only the Agreement needs to be resubmitted to
Medicare Australia.
(Note: The keys and certificates do not need to be revoked in this instance
because, as part of the Agreement, the HSE Rep submits a DAO Letter of
Authorisation that authorises the DAO to continue in their duty.)
