Pulmonary Rehabilitation Program

Program Description

Education, information & support to individuals with respiratory disease, through access to Nurse, Dietician, Physiotherapist, Group based pulmonary rehab program. A number of sessions are held throughout the year in various community based venues across Cairns & Hinterland and surrounds. The program is particularly suited to respiratory clients who are interested in attending group education and support sessions

Eligibility Criteria

A clinical diagnosis of COPD (an essential criterion) and may also have one more of the following: poorly controlled respiratory disease; impaired self-management capacity; history of medication non-adherence.

Chronic Disease Self Management Program (CDSM) Program

Program Description

Chronic diseases ~ What are they? What can I do? (6 x 2 hour sessions)

Eligibility Criteria

Target group are those people with pulmonary and/or cardiovascular conditions. Clients with other conditions may be considered on a case-by-case basis.

Important Information

The aim of the CDSM Program is to support the development of skills in self-management and personal goal setting for those people living with chronic conditions.

Home Based Cardiac Services

Description

Education, information & support to individuals with cardiac conditions through access to: Nurse, Physiotherapist, Psychologist, and Dietician

Eligibility Criteria

Those referred for cardiac rehabilitation at the Cairns Hospital

Important Information

This is a home based cardia rehab service for those unable to access centre based services
