Services and programs delivered by Cairns Community Health, fall under one or more of these 4 key domains:

Health promotion, protection and illness prevention

Includes services and programs which focus on: the promotion of the concept of wellness as a measure of health; the early identification of and intervention for people ‘at risk’ of developing chronic conditions; the prevention of illness and disease; men’s and women’s health.

Healthy Children, Young People and Families

Includes services and programs which focus on: the monitoring of child health development and well-being; health promotion and disease prevention; support for parenting; early identification of and intervention for ‘at risk’ individuals and families; maternal health; adolescent health.

Supporting the management of chronic disease and complex care

Includes services and programs which focus on: care coordination; support for self-management; case management (in partnership with the client’s GP); multi-disciplinary team support; cardiac and pulmonary rehabilitation; community-based rehabilitation; and lifestyle modification programs.

Healthy Ageing

Includes services and programs which focus on: interventions designed to promote the health, functioning, independence and well-being in older age; this includes the maintenance and management of older person’s health; dementia care and health care needs assessments.
