Stoke Outreach
Program Description
Offers education, information and support to people who have had a stroke. This is done through access to: Stroke Outreach Coordinator (currently Occupational Therapist); Stroke Outreach Rehabilitation Assistant. The aim of the program is to reintegrate clients into the community and to help prevent the sequelae of stroke (including readmission to hospital).
Eligibility Criteria
Clients who have a clinical diagnosis of stroke or Transient Ischaemic Attack (TIA)
Important Information
Clients can be referred by their GP or they may self-refer. The Stroke Outreach Service provides an in-reach service to Cairns Hospital and works closely with the hospital rehabilitation team to facilitate discharge home where appropriate
