Patient access to health records

The Darling Downs Hospital and Health Service (DDHHS) treats the information you provide to us in a confidential way as required under the Hospital and Health Boards Act 2011 and the Information Privacy Act 2009.

The physical health record is the property of the DDHHS however we recognise that the information you provide to us is your information. You may apply for access to your health record under the Administrative Access Scheme.

What to include with your application

You will need to provide a document establishing your identity before your application will be processed, as the DDHHS has an obligation to ensure that personal health records are only released to an appropriate person.

If you apply in person it will be sufficient for you to show an original form of identification such as a drivers licence, 18+ card, passport or birth certificate.

If you apply by mail you will need to provide a copy of your identification document and this copy must be certified as a true copy of the original by a Justice of the Peace or Commissioner for Declarations.

You should also include any change of name documents if your health records are in a different name to your identification documents.

Will I get all of my information?

If your application is able to be processed under the Administrative Access Scheme you will be provided with all of the documents you have asked for.

In some cases we will be unable to process your application under the Administrative Access Scheme due to the nature or content of the documents. Should this occur your application will be referred to a Right to Information/Information Privacy Decision Maker who will be in contact with you to discuss your application.

Access to information apart from your health record

If you wish to access information held by the DDHHS that is not your personal health record you will need to make a different type of application. This will include situations where you may wish to apply for your child’s health record or a deceased relative’s health record.
